Valsartan (trade names include Diovan, Avasart, Valsartan Teva, Nortivan, Valsartan Actavis, Provas, Valsartan Cinfa Laboratorios, Sarteg, Valsartan Ratiopharm, Tareg, Valsartan Mylan, Valpression, Valsartan Stada, Valtensin, Valsartan Sandoz, Valzaar) is a medication for cardiovascular system from Angiotensin II antagonists pharmacological group. This medicine is used to treat high blood pressure (HBP, hypertension) in adults and children, heart failure in adults, diabetic kidney disease. It is also prescribed for patients with post-myocardial infarction who have had a heart attack to improve the chance of living longer after a heart attack or myocardial infarction in adults. Valsartan works by blocking a chemical called angiotensin II, which causes blood vessels to narrow, restricting blood flow. Diovan also relaxes and widens blood vessels, allowing blood to flow more easily, reducing high blood pressure and stress on a heart.

    Indications and usage:

    Diovan (Valsartan) is an angiotensin II receptor blocker (ARB) indicated for:

  • Hypertension, to lower blood pressure in adults and children 1 year and older. Lowering blood pressure reduces the risk of fatal and nonfatal cardiovascular events, primarily strokes and myocardial infarctions.
  • Heart failure (NYHA class II-IV), to reduce hospitalization for heart failure in adults.
  • Post-myocardial infarction, for the reduction of cardiovascular mortality in clinically stable patients with left ventricular failure or left ventricular dysfunction following myocardial infarction in adults.


    Dosage and administration:

    Diovan (Valsartan) 40 mg, 80 mg, 160 mg, 320 mg tablets for oral use.


    Overdosage:

    Limited data are available related to overdosage by valsartan in humans. The most likely manifestations of overdosage would be hypotension and tachycardia; bradycardia could occur from parasympathetic (vagal) stimulation. Depressed level of consciousness, circulatory collapse and shock have been reported. If symptomatic hypotension should occur, institute supportive treatment.

    Diovan (Valsartan) is not removed from the plasma by hemodialysis.

    Valsartan was without grossly observable adverse effects at single oral doses up to 2000 mg/kg in rats and up to 1000 mg/kg in marmosets, except for salivation and diarrhea in the rat and vomiting in the marmoset at the highest dose (60 and 31 times, respectively, the MRHD dose on a mg/m2 basis) (Calculations assume an oral dose of 320 mg/day and a 60-kg patient).


    Contraindications:

    Known hypersensitivity to any component. Do not coadminister aliskiren with Diovan in patients with diabetes.


    Warnings and precautions:

  • Observe for signs and symptoms of hypotension.
  • Monitor renal function and potassium in susceptible patients.


    Adverse reactions, side effects:

  • Hypertension: most common adverse reactions are headache, dizziness, viral infection, fatigue and abdominal pain.
  • Heart failure: most common adverse reactions are dizziness, hypotension, diarrhea, arthralgia, back pain, fatigue and hyperkalemia.
  • Post-myocardial infarction: most common adverse reactions which caused patients to discontinue therapy are hypotension, cough and increased blood. creatinine

    Drug interactions:

  • Potassium-sparing diuretics, potassium supplements or salt substitutes may lead to increases in serum potassium, and in heart failure patients, increases in serum creatinine.
  • Non-steroidal anti-inflammatory drug (NSAID) use may lead to increased risk of renal impairment and loss of antihypertensive effect.
  • Dual inhibition of the renin-angiotensin system (RAS): Increased risk of renal impairment, hypotension, and hyperkalemia.
  • Lithium: increases in serum lithium level and lithium toxicity.

  • Lactation: breastfeeding is not recommended.
  • Pediatrics: use of Valsartan (Diovan) is not recommended in children less than 1 year of age.
